How Much Will I Pay?
Your cost will depend on the federal poverty guidelines, which are updated annually in January.
Based on your household income and family size, you may qualify for primary care visits starting as low as $30.

When you arrive at your visit, let the receptionist know you would like to apply. You can start the application on the check-in Kiosk as well. For your convenience, the application can be printed here:
You must know the number of persons in your household and bring proof of income.
Your fee will be based on the level of discount determined by household size and income. It may be as low as $30 for primary care services.
Annual poverty guidelines are updated in January of each year.
We offer payment plans; call our Billing Department to arrange payment plans if needed. We can also help you enroll in Medicaid, Medicare, and Insurance through the Affordable Care Act Marketplace.
